Types of Application
ICCA offers a range of registration
for ICCA'acupuncturists, covering Regular, an
apprentice student, non-practising,

To register, you must send in a signed application form, the examination
fee and documents
1. Regular
Application - for new graduates or
already graduation
The requirements
for Regular Application are :
Either 1.1.1 graduation from an acupuncture education or training program
approved by the
ICCA Registration Association; and
1.1.2 successful completion of the examinations required by the ICCA
Registration
Association; and
1.1.3 evidence satisfactory to the Registration Association of the good
character of the
person consistent with the responsibilities of a registrant and the
standards expected
of a registrant;
2. or
2.1.1 has a combination of knowledge, skills and abilites which are,
in the opinion of the
ICCA Registration Association, substantially equivalent to the requirements
established in subsection 1.1.1 - 1.1.3; and
2.1.2 has successfully completed the examinations and courses required
by the ICCA

3. Practice
Experiences Application
3.1 has been practising in a capacity substantially
equivalent to a registrant for any 12 month period during the 18 monthes
prior or 24months subsequent to the effective date of the Private Institute,
i.e., a minimum of 120 patient visits in a consecutive 12-monthe period;
and has never been qualified, and dose not at the of application, qualify
; and
3.2 has successfully completed any courses and courses and examinations
required by the ICCA Registration , including a College mandatory upgrading
course; and
3.3 satisfies the ICCA Registration Association concerning the good
character of the person consistent with the responsibilities of a registrant
and the standards expected of a registrant; and
3.4 obtain a minimum of 70 points based upon the following standards
and criteria:
3.5.1 practice in a capacity substantially equivalent to a registrant
for any 12 month
period not concurrent with the 12 months declared in 3.1, i.e., a minimum
of 100
patient visits in a consecutive 12-month period 15 points per 12 month
period
3.5.2 practice outside of non regular institute in a capacity substantially
equivalent to a registrant for a 12 month period not concurrent with
the 12 months declared in 3.1, i.e., a minimum of 120 patient visits
in a consecutive 12-month period 5 points per 12 month period
3.5.3 education program(s) acceptable
to the ICCA Registration Association , i.e., any course of study containing
both clinical and theoretical components, based on a single program
or integrated curriculum containing content nesessary for entry level
acupuncture practice
max. 25 points per yearof study
3.5.4 apprenticeship program(s) acceptable to the ICCA Registration
, i.e.,
supervised practice with a tutor or preceptor who assumes responsibility
for the theoretical and practical education and training of the apprentice
max. 45 points per year of study
3.5.5 acupuncture licensure in another
jurisdiction acceptable to
the ICCA Registration Association
20 points
3.5.6 other professional activities acceptable to the ICCA Registration

4. Struedent Application
An applicant may be grated
student registration by the Registration Association where the application:
4.1 is enrolled, or was enrolled during the 8 months previous to making
an application as a student in a training program approved by the ICCA
Registration ; and
4.2 satisfies the ICCA Private Committee concerning the good character of the
person consistent with the responsibilities of a registrant and the
standards expected of a registrant.
4.3 A person must be registered as a student registrant before undertaking
a period of practical training involving direct patient care.
5. General Information
-Document
Requirements
For ICCA to validate your acupuncture credentials,
you will need to submit formal, official, educational documents for
all of your fully or partially completed credentials. Official documents
are original
documents (not photocopies) issued by an educational institution. They
bear a seal or signature to indicate authenticity. In addition to the
documents requested for all types of registration, you must also submit
a datailed syllabus of each course you completed to earn your credential.
You will also need to provide original
documents for all your acupunctrue practices and professional activities.
Notarized statements verifying year and number of patient visits must
accompany the application.
Translations
If any of your original educational documents
are not in English , they must be accompanied by a full and complete
word-for-word (literal, not interpretive) English /translation completed
by Certified translator.
Credentials such as mark sheets, transcripts,
diplomas, certificates, and grade books must be translated in entirety
(including any information that appears on the reverse side of any document).
In the case of calendars, course outlines, and syllabuses, only those
pages describing courses and other information specific to your credential
need to be translated.
Translations may only be conducted from
original, official documents. The translator must attach to each translation
a singed copy of the educational documents on which the translation
is based. The document copy or copies accompanying the translation will
be verified against the original, official documents submitted to ICCA
. All document copies submitted for validation must also be submitted
to ICCA.

Registration Processing Time
A Certificate may be issued by ICCA after
completion of the evaluation process. Pleas note that applications that
require unusual research or documentation may take longer to complete.
ICCA will confirm the receipt of your application by mail within two
weeks of receiving it.
Cancellations and Refunds
A 50% refund will be given if ICCA receives your written cancellation
request before all documents have been received and evaluation work
has begun.
Refunds will not be issued
1. if ICCA finds that any educational
documents or translations submitted have been altered or are fraudulent.
2. if ICCA finds that the translation of a document falsely represents
the original.
3. if information included in an application is found to be fraudulent
or misrepresented.
4. if you cancel a request for registration after an evaluation has
commenced.
Refunds will be issued in Canadian funds
and will be sent by regular mail to your name only.

Type of Application
Choose the type of registration that
will best meet your needs. Where applicable,
place a check mark in Education:
Education means any course of study,
containing both clinical and theoretical components,
based on a single program or integrated curriculum, containig content
necessary for entry level acupuncture practice. ICCA requires information
about all institutions you have attended to study acupuncture, TOM or
HOM health relate education/training and any education redentials (such
as certificates, diplomas, degrees) you have obtained. Use one line
for each credential you have fully or partially completed. Original
documentation and proof of completion of the education undertaken must
accompany the application, e.g., original school diplomas or certificates;
original school official transcripts showing grades, hours and subjects(must
be sent directly from the school to the ICCA) original school curriculum
and course descriptions including content
and number of hours for each course; ¡¦ect.
Apprenticeship
Apprenticeship means supervised practice
with a tutor or preceptor who assumes responsibility for the theoretical
and practical education and training of the apprentice. ICCA requires
information about all institutions you have attended for your apprenticeship
(including acupuncture, TOM, HOM health related apprenticeship training)
and any apprentices's qualifications and nature of practice (such as
acupuncture, TOM,HOM etc.) you have obtained. Use one line for each
credential you have fully or partially completed. Original documentation
and proof of completion of your apprenticeship must accompany the application,
e.g., notarized letter from preceptor or tutor that describe the apprenticeship
(including the date of start and finish, data of start and finish, total
number of hours, amount of time spent in theoretical instruction, level
of involvement of apprentice with patient treatments; preceptor's practice,
numeber of patient visits per year, type of practice, ¡¦etc.); original
certificate from preceptor or tutor; notarized copy of your application
for apprenticeship from the regulatory agency under which you worked.
Practice
Practice means practising within the
scope of practice as defined in section 4 of the Acupuncturists Regulation
under the Health Professions Act, during a minimum of 120 patient visits
in a consecutive 12-18month period. Original documentation, and notarized
statements verifying year and number of patiect visits must accompany
the application. The documentation may be accomplished in a variety
of ways, e.g., notarized copies of name, address records/appointment
calendars /journal sheets/leases/business permits; original letters
from employers giving dates and hours worked and nature of practice
and number of patient visits; affidavits from a minimum of thirty patients;
affidavits from other health care professionals.
Other Activities
You must provide details of other professional
activiites that you have paricipated in, such as professional association
membership, any testing or credentials required for the membership,
teaching experiences, conferences, seminars of workshops, etc.. Original
documentation supporing each of the activities must accompany the application.
Mandatory Education Course
ICCA requires the information about the
date and location of the mandatory education course you undertook. Original
documentation verifying your attendance must accompany the application.
Disclosrue of Complaints/Discipline/Claims
You must list all activities relating
to any acupuncture or health-related practice which have resulted in
any ethical/professional/other complaints, dismissals from associations/positions/¡¦ets.,
fines, professional discipline, criminal prosecutions/civil suits, or
any other formal complaints. Original documentation of completion of
remedial activities taken must accompany the application.
Language Spoken
Choose the level of proficiency for each
language and circle your choice.
Document Return Order
After all required documents have been
received and examined by ICCA , they will be returned to you. Please
indicate how you would like to have your documents returned. We recommend
that you choose courier service to ensure the safety of your documents.
The appropriate fee must be included with your application if you wish
to have your documents returned to you by courier.
Total Fees
Addall the fees from each category and
enter the total in the space provided.
Payment Method:
Please select the method of payment you prefer. Acceptable methods of
payment are: cheque and money order. All payments must be made in funds
and cheques must be drawn on banks. Payment should be made to the 'IPE
of ICCA'.
Applicant's Statement
Please read carefully the terms and conditions.
You must sign and date the ICCA application form. Your signature indicates
that you agree with the Applicant's Statement. Your application form
will be returned to you, unprocessed, if it is incomplete or if it is
not signed by you.
Proof of Professionalism
You must obtain the signatures of two
persons who are licensed acupuncturists or members of a recognized professional
society, attesting that you are capable of meeting the Oriental Medicine
& Acupuncture Association of ICCA's Good Character criteria.
